IEOR E4999: Fieldwork Request Form (For MS Students Only)
IEOR E4999 Fieldwork is intended for IEOR graduate students interested in obtaining relevant professional work experience as part of their program of study. IEOR graduate students may earn up to (3) three graded credits towards the M.S. degree through internship engagement. The student is responsible for the cost per credit, charged as regular tuition. Students and supervisors will be required to submit a final report at the conclusion of the internship.
In order to commence, the student must adhere to the following guidelines:
1. Student must present an official offer letter from the employer stating the following four items. The offer letter must contain all the items below.
I. Beginning and ending dates of employment.
II. Number of hours of employment per week; employment may not exceed 20 hours per week during the fall and spring semesters.
III. Description of internship duties and responsibilities in detail to demonstrate its relevance to your program of study.
IV. Location of the internship.
2. Student must adhere to the Fieldwork schedule and deadlines.
The Department require 3-5 business days to review and process your Fieldwork request. For F-1 visa holders, ISSO will require an additional 5-10 business days. Please communicate this time frame to your prospective employers.
